During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Discussed not allowing hot beverages around children, fish tank products, tracking 16 hours of annual training, model orientation form, model reference form, background check details, lead qualifications, and child abuse training.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 11/12/2025. The on-site inspection began at 10:55am and ended at 12:55pm. The inspector reviewed compliance in the areas listed above. There were twenty-three children present and four staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-1
Description: The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for Staff #1, who is currently working, was not found during the inspection.

Plan of Correction: The center will have the staff complete the sworn disclosure statement

Standard #: 8VAC20-770-60-C-2
Description: For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service. Caregiver #1 did not have central registry search results during the inspection and has been employed for over five months.

Plan of Correction: The center will obtain the central registry search for the staff member.

Standard #: 8VAC20-780-160-A
Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 had no documentation and had been employed for five months.

Plan of Correction: The center will obtain a TB screening for the staff.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances must be in a locked place that prevents access by children. Bleach was found in an unlocked cabinet in the main classroom, out of reach of children.

Plan of Correction: The center will have child locks installed on the cabinet.

Standard #: 8VAC20-780-560-G
Description: When food is brought from home the food container must be clearly dated. Several of the children's food containers in the center were not dated.

Plan of Correction: The center will implement a new system to ensure that food brought from home will be labeled with the date.
