During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated and completed on 11/21/25. The on-site inspection began at 10:50am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 53 children present and 11 staff. The inspector reviewed 5 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-E Description: Repeat Violation
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The record for child 1, who is under the age of 2 years and has been attending the center for over a year, does not contain documenation of additional immunizations.Plan of Correction: The center responded with the following: The parent will be contacted today and an updated immunization record for child 1 will be requested. Once obtained, it will be placed in the child's record.
Standard #: 8VAC20-780-510-G Description: Medication shall be labeled with the child's name. Medication for child 2 was onsite, but was not labeled with the child's name. Plan of Correction: The center responded with the following: Child 2's parent will be contacted today for them to bring in the medication's original container. Going forward, center management will ensure that all medication being stored at the center is kept in the original container.
