An unannounced, on-site renewal inspection was initiated and completed on November 21, 2025, as a part of the licensure period. The on-site inspection began at 10:00 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 21 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 2, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-3 Description: The center must request a search of the central registry prior to the employee's first day of employment.
Staff #1, who had been employed for over 1 year, did not have a central registry search requested.Plan of Correction: All staff will have central registry request completed.
Standard #: 22.1-289.035-B-4 Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff #1, employed for over 1 year, did not have documentation of requesting a central registry check, criminal history check, or sex offender check from one state.Plan of Correction: Out of state background check will be completed.

Standard #: 8VAC20-780-80-A Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
The main attendance record for the center did not have two children recorded when they arrived. The attendance record in Classroom #1 did not have two children recorded when they arrived.Plan of Correction: Daily attendance will be maintained as a written record accurately.
Standard #: 8VAC20-780-240-A Description: Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff #3, who is working with children and employed over 1 year, has not yet completed this course which is overdue by 9 months.Plan of Correction: All staff will complete the VDOE orientation.
Standard #: 8VAC20-780-500-A Description: Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.
Staff # 1 changed a child?s diaper in Classroom #3 and did not wash the child's hands with soap and water.Plan of Correction: All childrens hands will be washed with running water at diapering.
Standard #: 8VAC20-780-560-G Description: When food is brought from home the food container must be clearly dated.
The food container for Child #7 in the main refrigerator were not dated.Plan of Correction: All food containers will be dated.
