During the inspection, the licensing representatives reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.?

Comments:
An unannounced, onsite monitoring inspection was initiated on 11/19/2025 and completed on 11/24/2025. The on-site inspection began at 9:55 am and ended at 12:20 pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/03/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4
Description: The center is required to obtain background checks from any state in which the individual has resided.

The center has not obtained a child abuse and neglect check from another state for Staff 4, who has been working at the center for more than 1 year and was working alone with a group of children.

Standard #: 8VAC20-780-80-A
Description: REPEAT VIOLATION

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were 10 children present in Classroom 2; there were only 9 children marked present on the attendance record.
There were 11 children present in Classroom 3; there were only 9 children marked present on the attendance record.

Description: The center shall provide the license representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The annual fire inspection report was 5 months overdue.

Standard #: 8VAC20-780-270-A
Description: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

The push-bar on the exit door in Classroom 4 was missing. A metal spring was used to prevent the door from opening. The end of the metal spring was sharp point and at eye level for small children.
There was mold growing on the spigot and ceiling in Restroom 1.

Description: Cleaning materials and insecticides/pesticides shall be stored in areas physically separate from each other.

There was a container of Raid Ant & Roach Spray stored in a cabinet with the cleaning supplies in Classroom 2.

Plan of Correction: Item was removed from the building all together and all staff informed on how to properly store it.

Description: In each group of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Staff 2 was the only staff member supervising Classroom 2. Staff 2 does not meet the qualification of a program leader or program director.

Description: The required ratio for children 2 years old is one staff per 8 children.

Description: The diapering surface shall be cleaned with soap and water and sanitized after each use.

Staff 2 did not clean and sanitize the diapering surface after each use.

Description: No child shall be allowed to drink or eat while walking around.

Multiple children in Classroom 1 were walking around the classroom while drinking from sippy cups.
