During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. An unannounced monitoring inspection completed on 11/17/2025. The on-site inspection began at 10:25 and ended at 12:30. The inspector reviewed compliance in the areas listed above. There were 21 children and 5 staff present. The inspector reviewed 5 children?s records and 5 staff records. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Description: At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
Staff #5 repeat TB screening was overdue by about 5 weeks.Plan of Correction: The staff person will get their repeat TB screening.
Standard #: 8VAC20-780-550-D Description: The center shall implement a monthly practice evacuation drill.
The October drill was not completed.Plan of Correction: Two drills will be completed in November.
