During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on and completed on 11/13/2025, as a part of the licensure period. The on-site inspection began at 10:30am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 51 children present and 17 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #5, enrolled for more than one year, did not contain the name, address, and phone number for 1 of 2 designated people to call in an emergency if a parent cannot be reached; Plan of Correction: Administration emailed the parents to request the information during the inspection.
Standard #: 8VAC20-780-70 Description: Centers are required to maintain a separate record for each staff person which contains all required information. The record for Staff #3, a lead teacher who has been working at the center for over 10 months, did not contain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position. Plan of Correction: The center will review staff's training to determine qualification and ensure all supporting documents are in the staff's record.
