During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced inspection was initiated and completed on 11-24-25 with a local agency in response to a complaint received by the licensing office on 11-19-25 relating to allegations of inadequate supervision, rough handling of a child, and the completion of reports in a timely manner. This inspection included document review, interviews, and observations. The preponderance of evidence gathered during the investigation supports one of the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12-2-25. A POC submitted after this date will not appear on the public website.

Description: The Center is required to ensure that the written record of children?s injuries contained the staff present, the date and time when parents were notified; any future action to prevent recurrence of the injury; and staff and parent signatures or two staff signatures.

After reviewing 3 injury reports for Child #1, it was determined that one of the injury reports written for Child #1 was missing documentation of the staff present and a second signature. 2 reports were missing the date and time the parents were notified and any future action to prevent recurrence of the injury.

Plan of Correction: All center staff have been retrained on filling out an incident/accident report. Staff have also been given a sample to make sure they understand all areas that must be filled out
