Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on July 5, 2017 from 1:15 p.m. through 3:30 p.m. On this date, there were forty-nine (49) children in the care of ten (10) staff members. Nine (9) children's records and ten10) staff records were reviewed and updated at the YMCA's main office from 10:00 a.m. through 1:00 p.m. The medication and medication paperwork was reviewed for four children. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were hand washing/bathroom procedures, transition, arts and crafts, and indoor organized play. All interactions between staff and children were appropriate and the children seemed comfortable with the daily routine. Standard #: 22VAC40-185-510-C Description: Based on review of medication, the center failed to follow it's procedures. Evidence: 1. One Epi- Pen reviewed had an expiration date of 03/2017. 2. The site director confirmed the date. 3. One Albuterol inhaler was in the medication locked box. 4. The parent authorization to administer the medication had expired. The authorization was signed 04/21/2017. 5. The site director confirmed the authorization was dated 04/21/2017. Plan of Correction: The medication will be returned to the parent. If the medication is still needed, a new parent written authorization will be requested. Medication policies will be reviewed with MAT trained staff.
