During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on October 23, 2025, and completed on November 19, 2025, in response to a self-report received by the licensing office on October 23, 2025, relating to Forbidden Action. The inspector reviewed one child?s record and two staff records on-site. This inspection included document review tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non- compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the
close of business on December 4, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-410-1 Description: The following actions or threats thereof are forbidden; roughly handling a child. Evidence to support the finding:
1. On October 21, 2025, Child #1 reported Staff #1 ?grabbed me by the wrist and threw me on the ground.?
2. Staff #2, the Director, observed Staff #1 holding Child #1?s arm and did not have patience stating, ?He?s not listening.?
3. Child #1 hit the floor with his elbow when he fell.Plan of Correction: The center responded with the following: Staff #1 was terminated during the investigative process and will no longer work for Champions. Current Staff will continue use appropriate redirection for children in care and follow all Licensing Standards for Virginia.
