During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 11-17-2025 and completed on 11-26-2025. The on-site inspection began at 11:05 am and ended at 12:38 pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records electronically from 11-20-2025 through 11-126-2025. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
12-5-2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information. The record for Child #2 and Child #3 did not include the employment location and phone number for one parent listed.

Standard #: 8VAC20-780-70
Description: Staff records for each person shall include all required items. Staff #1 was missing documentation to demonstrate they possess the experience and education required by the job position. Staff #1 is designated as program director.

Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. The attendance sheet for classroom E was not accurate as it only listed 5 children in care and there were 12.

Description: The areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There were 6 large bolts protruding from the playground boarder.

Description: A 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone. The required phone numbers were not posted in 5 out of 5 classrooms (classrooms, A,B,C,D and E) observed.
