During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was conducted on November 24, 2025. The on-site inspection began at 4:15 p.m. and ended at 6:15 p.m. The inspector reviewed compliance in the areas listed above. There were 11 children present and 7 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: At least every two years from the date of the first initial screening or testing, staff members shall submit the results of a follow-up tuberculosis screening. Staff 2 did not have a current tuberculosis test or screening; the documented test expired 2 months ago.

Plan of Correction: TB tests and screenings will be repeated every 2 years as required.

Standard #: 8VAC20-780-60-A
Description: The facility shall have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child 1 has diagnosed food allergies but did not have a written allergy action plan on file.

Plan of Correction: An allergy action plan will be requested from the parent.

Standard #: 8VAC20-780-240-I
Description: Documentation of orientation training shall be kept by the center in a manner that
allows for identification by individual staff member, is considered part of the staff
member's record, and shall include: the date of training. Orientation for Staff 3, employed for almost 3 months, was not dated.

Plan of Correction: The orientation form will be completed as required.

Standard #: 8VAC20-780-260-A
Description: The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The last documented fire inspection expired 4 months ago.

Standard #: 8VAC20-780-550-E
Description: Shelter in place procedures shall be practiced a minimum of twice per year. The last documented shelter in place drill was 1 year and 4 months ago. Staff confirmed the drills had not been conducted.

Description: Lockdown procedures shall be practiced at least annually. The last documented lockdown drill was 1 year and 4 months ago. Staff confirmed the drill had not been conducted.
