During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated on 10/29/2025 and completed on 11/23/2025 (in response to a complaint received by the licensing office on 09/15/2025 relating to allegations to supervision and a child?s injury. The inspector reviewed one child?s record, and one staff record on-site. This inspection included document review tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/05/2025. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Staff #1 observed that the fire extinguisher attached to the wall near the child-size sofa in Classroom #1 was ?very loose and hanging?. Staff #1 heard Child A (two years old) cry out. The fire extinguisher had fallen off the wall and struck Child A on the side of the face and mouth. Child A had a red mark on the cheek and was bleeding from the mouth. The parent was called to pick up Child A.Plan of Correction: Immediate action taken. Fire extinguisher was relocated and secured appropriately 9/15/2025. Management will conduct daily checks to ensure fire extinguishers are secure.

When staff are supervising children, they shall always ensure their care, protection and guidance.
Staff #1 reported telling the previous supervisor twice about the loose, handing fire extinguisher. The supervisor said a work order would be submitted, but the fire extinguisher was not repaired. The child-size sofa remained next to the fire extinguisher that was not properly secured to the wall.Plan of Correction: Immediate action taken. Fire extinguisher was relocated and secured appropriately 9/15/2025. Moving forward, management will ensure that areas are safe, and work orders are placed immediately with a follow up.

The center shall maintain a written record of children?s serious and minor injuries in which entries are made the day of occurrence.
Staff #1 and a collateral witness both stated that Child A was injured while under the supervisor of the childcare center. A written record of the injury Child A sustained could not be located.Plan of Correction: Management retrained staff on proper incident reporting on November 3-7, 2025 and again 12/10/2025 as a refresher. Teachers contact management when there is an incident, complete an Incident Form in its entirety, and give to management. Management will review form and ensure it is completed properly. Management to obtain signature from parent.
