Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.?

Comments:

An unannounced, on-site monitoring inspection was initiated on 11/13/2025 and completed 11/25/2025. The onsite inspection began at 3:00 pm and ended at 4:15 pm. The inspector reviewed compliance in the areas listed above. There were 24 children and 3 staff. The inspector reviewed 5 children?s records on-site and 5 staff records electronically. The inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-780-160-A
Description:	REPEAT VIOLATION Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment and shall have been completed within the last 30 days of employment. There was no documentation of a TB screening for Staff 5, who has been employed for 21 months.
Plan of Correction:	The staff will get it in the next 30 days.
Standard #:	8VAC20-780-280-H
Description:	Harmful agents shall not be stored in areas that are accessible to children. There was a set of keys, with a canister of pepper spray attached to them, laying on the front table of the classroom. Children, ages 3-4 years, were observed in this area of the classroom, and the set of keys was accessible to the children
Plan of Correction:	Will make sure the keys are locked up.

Description:	REPEAT VIOLATION The required ratio for children 3 years old up to school age eligible is one staff per 10 children. There were 2 staff members supervising 24 children, ages 3 years-4 years, for approximately 35 minutes; 3 staff were required for this group of children.
Plan of Correction:	Will make sure that all staff are present and on time .

Description:	Tables shall be sanitized before and after each use for feeding. The tables were not sanitized after being used for the afternoon snack. Children immediately resumed normal activities at the tables following the afternoon snack.
Plan of Correction:	Will sanitize after they eat.
