An unannounced, on-site monitoring inspection was initiated and concluded on November 20, 2025. The on-site inspection began at 10:43 a.m. and ended at 1:30 p.m. The inspector reviewed compliance in the areas listed above. There were 35 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 9, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-240-A Description: Repeat Violation
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #2, who is working with children, has not yet completed this course which is overdue by two weeks.Plan of Correction: Employee has been given 5 days to complete the training.
Standard #: 8VAC20-780-510-B Description: Systemic Violation
Prescription and nonprescription medication shall be given to a child only with written authorization from the parent. Child #5, enrolled for three months, requires an emergency medication and the center did not have written authorization from the parent to administer the medication.Plan of Correction: The student's parents were notified of the incomplete form and they came in to complete it. Staff will begin to review paperwork upon receipt to ensure it is completed before the parent leaves.
Standard #: 8VAC20-780-510-G Description: Systemic Violation
Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Medication for child #4, enrolled for three months, was not labeled with the dosage amount and the time or times to be given.Plan of Correction: Professional development will be given to staff to ensure an understanding of how medicine should be labeled, especially when the prescribed dosage is different from the packaging instructions.

Medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
1. In the Rosebud classroom, medication was in an unlocked bag.Plan of Correction: The Rosebud classroom has been equipped with a locking medicine bag.
Standard #: 8VAC20-780-510-M Description: Systemic Violation
If a key is used for a medication box, the key shall not be accessible to the children. In the Sunflower class, the key to the medication box was inserted into the locking mechanism of the medication box and it was accessible to children.Plan of Correction: The Sunflower staff have labeled the key and put it in an area that is accessible but out of children's reach.
