An unannounced, on-site monitoring inspection was initiated on 11/20/2025 and completed on 12/1/2025, as part of the licensure period. The on-site inspection began at 9:07am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 10 children and 2 staff present. The inspector reviewed on-site records of 3 children on 11/20/2025. Staff records were reviewed off-site on 12/1/2025. Medication for 1 child was reviewed on-site on 11/20/2025.
The inspection included the following: document reviews, tour of the facility, and observations.
