An unannounced inspection was initiated on 11/20/2025 and completed on 11/24/2025 in response to a complaint received by the licensing office on 11/17/2025 relating to allegations of administration, physical plant, staffing supervision and programs. This inspection included document review, tour of the facility, interviews, and observations. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
