During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on November 25 2025 and completed on November 25 2025. The on-site inspection began at 10:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 69 children present and 12 staff. The inspector reviewed 4 children?s records and 4 staff records. This inspection included a document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program..
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 11 2025. A POC submitted after this date will not appear on the public website

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Documentation of viewing proof of the child's identity and age.
The center did not ensure the viewing of proof of the children's age and identity was documented. Child #1, #2, #3, and #4 did not have documentation on record that their proof of identity and age was reviewed. The center is 8 months out of compliance.Plan of Correction: per an inspector visit and directions; list of missing ID's will be reported to FFX police.
Standard #: 8VAC20-780-560-G Description: When food is brought from home, the following shall apply: The food container shall be sealed and clearly dated and labeled in a way that identifies the owner; Plan of Correction: Lunch labels and tags added to all food containers.
