During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on 12/04/2025 and completed on 12/04/2025. The on-site inspection began at 9:22am and ended at 11:08am. The inspector reviewed compliance in the areas listed above. There were 12 children present and four staff. The inspector reviewed five children?s records and four staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Repeat Violation:
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
Child #3, who had been enrolled for 12 months, did not have a physical examination on record at the time of inspection.
Child #4, who had been enrolled for 12 months, did not have a physical examination on record at the time of inspection.Plan of Correction: Will have parents submit within 48 hours.
Standard #: 8VAC20-780-150-B Description: Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
Child #5, who has been enrolled for over 2 months, had a physical and immunization that was not signed and dated by a physician, his designee, or an official of a local health department.Plan of Correction: Will have parents submit within 48 hours.
Standard #: 8VAC20-780-60-A-8 Description: A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Child 1 has a diagnosed food allergy and there was no allergy care plan from the physician, which was overdue by three months.Plan of Correction: Will have parent submit within 48 hours.
Standard #: 8VAC20-780-270-A Description: Repeat Violation:
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
In classroom 1, a part of the wall had chipping paint and cracked and broken drywall exposing metal edges.Plan of Correction: Will have maintenance repair this within 48 hours.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In classroom 1, two cans of cleaning materials were observed on the counter unlocked and accessible to children.Plan of Correction: Corrected on site.
