Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated on 12/02/2025 and completed on 12/08/2025. The on-site inspection began at 12:15 p.m. and ended at 2:30 p.m. Two inspectors reviewed compliance in the areas listed above. There were 8 children present and 3 staff. The inspectors reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (12/15/2025). A POC submitted after this date will not appear on the public website.

Description:	SYSTEMIC VIOLATION The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunization for Child 2 (enrolled three months) was obtained one day after the child first attended the center and Child 5 (enrolled one month) did not have documentation of an immunization on file.

Description:	SYSTEMIC VIOLATION Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There were no physical examinations in the records of Child 2 (enrolled for three months), Child 3 (enrolled for seven months) or Child 5 (enrolled for over 30 days).

Description:	SYSTEMIC VIOLATION Centers shall be in compliance with the center?s own policies and procedures required by these standards. The center?s medication policy states that medication will not be administered. Staff 3 administered medication to Child 4 on each day of their attendance during the months of June, July and August 2025.

Description:	REPEAT VIOLATION SYSTEMIC VIOLATION Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for Child 1, Child 2, Child 3, Child 4, and Child 5 were each missing several of the required components. The record for Child 1 was previously cited on 6/3/2025 for missing required components.

Description:	REPEAT VIOLATION SYSTEMIC VIOLATION Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. There were no references on file for Staff 1(employed over nine months) or Staff 2 (employed over two months).

Description:	SYSTEMIC VIOLATION For centers offering multiple shifts, a qualified program director or qualified back-up director shall regularly be on site at 50% of the day shift and at least two hours during the evening shift. During the evening shift, a qualified program director is not on site for the required two hours.

Description:	SYSTEMIC VIOLATION Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. There was no orientation training for Staff 2 who had been employed for over 2 months.

Description:	SYSTEMIC VIOLATION Prior to working alone with children and within seven days of the first day of employment, staff shall be provided in writing all of the required information. There was no documentation of required information provided in writing to Staff 2 who had been employed for over 2 months.

Description:	SYSTEMIC VIOLATION The administration of medication shall be performed by a staff member who has satisfactorily completed medication administration training. There is no documentation that Staff 1, who administered medication 1 to Child 4 on multiple occasions, has completed medication administration training.

Description:	During the center?s hour of operation, one adult on the premises shall be in charge of the administration of the center. This person shall be either the administrator or an adult appointed by the licensee or designated by the administrator. Upon arrival, staff were unable to identify the administrator on-site. The identified administrator arrived approximately five minutes later.
Plan of Correction:	An administrator will aways be assigned and onsite.

Description:	SYSTEMIC VIOLATION The medication authorization shall be available to staff during the entire time it is effective. There was no medication authorization available to staff during the time medication 1 was administered to Child 4 for approximately 3 months.

Description:	SYSTEMIC VIOLATION Prescription medication may be administered if the parent has completed a written authorization from a parent or guardian. There was no written parent authorization to administer prescription medication 1 to Child 4. Staff 3 administered medication to Child 4 on each day of their attendance during the months of June, July and August 2025.

Description:	SYSTEMIC VIOLATION Medication shall be kept in a locked place using a safe locking method that prevents access by children. Medication 1 was in an unlocked desk drawer in the entryway. The drawer was accessible to children.

Description:	SYSTEMIC VIOLATION Centers shall keep a record of medication given to children which shall include the required information. There was no record of medication 1 given to Child 4 on each day of their attendance during the months of June, July and August 2025.

Description:	REPEAT VIOLATION SYSTEMIC VIOLATION There shall be at least two staff members who have current certification in CPR and first aid present on the premises during when children are in care. Upon arrival, Staff 1 and Staff 2 were the only staff on premises. Staff 1 and 2 are not certified in CPR or first aid. Staff 3, who is certified in CPR and first aid arrived five minutes later. The center was previously cited on 6/03/2025 for not having staff certified.
