During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced on-site monitoring inspection was initiated and completed on 12/04/2025. The on-site inspection began 1:00 at the main office where 4 staff records were reviewed until 2:10. The inspection continued at the site where children were in attendance from 2:15-3:30. The inspector reviewed compliance in the areas listed above. There were 10 children and 3 staff present. The inspector reviewed 4 children?s records. This inspection included document review, tour of the facility, interviews, and observation.
Information gathered during the inspection determined non-compliance with the applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-420-E-3 Description: Staff shall request at least annually parent confirmation that the required information in the child's file is up to date.
There was no documentation that child record #1 and #4 had been updated annually and they had been enrolled for over 1 year.Plan of Correction: The parents will be asked to update the information and it will be documented. All records will be reviewed to make sure they have been updated and that will be documented.
Standard #: 8VAC20-780-560-J Description: Tables shall be sanitized before and after each use.
Staff stated tables were not being sanitized.Plan of Correction: Tables will be cleaned and sanitized after each use.
