An unannounced monitoring inspection was initiated on November 10, 2025 and completed on December 10, 2025 with a local agency in response to a self-report received by the licensing office on November 10, 2025 relating to supervision of children. The inspector reviewed 2 children's records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to operational responsibilities and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 17, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: The operational responsibilities of the licensee shall include ensuring that the center?s activities, services, and facilities are maintained in compliance with these standards, the center?s own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
1. The center?s playground safety procedures state ?staff should be positioned in a way that there are no blind spots and that staff should circulate around to prevent blind spots from occurring.? The playground map for staff indicates there should be staff at each corner of the playground. The center did not follow its own procedures when video footage showed three staff in the middle of the playground with their backs to children playing at the top corner of the playground. Infant #2 (age 16 months) walked around that corner and was out of sight and sound of staff #1, staff #2, and staff #3 for less than a minute.
2. The center?s procedures for identifying where attending children are at all times states, ?Children will always be within actual sight and sound supervision of staff.? It also states that staff are to scan the room and count children, especially during transitions, when children are moving from one location to another. The center did not follow its own procedure when 1.) Infant #2 (age 16 months) was out of sight and sound supervision of staff #1, staff #2, and staff #3 for less than one minute and 2.) Staff #1 and staff #2 left Infant #1 (age 14 months) alone and unsupervised in the classroom for five minutes when the staff took the rest of the class outside. Staff #1 and staff #2 did not do a scan of the room or correct count of the children in their care.Plan of Correction: #1 & #2 On November 10th and 11th, 2025 Program completed Mandatory meeting with all Program Staff sharing the events that took place. Immediate
re-training of policies and protocols reviewed with staff ensuring the understanding and mandated protocols to have in practice.

understanding of the Full Playground Plan and health, safety and assurance of constant supervision and care components.
#2 Program implemented a ?Face to Name? Written Form on a classroom Clipboard to use every time a grouping of children crosses a threshold to assure
constant supervision of all children. This listing will mirror the names and reconcile with Head Count.
#1 & #2 Quality Control Checks will monitor this daily, weekly and monthly as spot checks by supervisors to consistently maintain this standard.
Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. An infant was left unsupervised.
1. On November 7, 2025, staff #1 and staff #2 did not scan the room or correctly count the children in their care during a transition from the classroom to the playground. Infant #1 (age 14 months) was left alone inside the classroom without any staff supervision for five minutes.Plan of Correction: On November 10th and 11th, 2025 Program completed Mandatory meeting with all Program Staff sharing the events that took place. Immediate re-training
of policies and protocols reviewed with staff ensuring the understanding and mandated protocols to have in practice.

Standard #: 8VAC20-780-340-F Description: Children under 10 years of age shall always be within actual sight and sound supervision of staff. On November 7, 2025, two infants were unsupervised on two separate occasions.
1. While on the playground, infant #2 (age 16 months) exited the center's make-shift barrier and left the outdoor play area without staff knowledge and was unsupervised for less than one minute.
2. Staff #1 and staff #2 left Infant #1 (age 14 months) alone, inside the classroom unsupervised for five minutes.Plan of Correction: On November 10th and 11th, 2025 Program completed Mandatory meeting with all Program Staff sharing the events that took place. Immediate re-training
