An unannounced, on-site monitoring inspection was initiated on December 11, 2025 and completed on December 11, 2025. The on-site inspection began at 12:05 p.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 47 children present and 12 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, observations, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry or sex offender registry from one state for Staff #3, who has been employed for three years and is working with children. Plan of Correction: The sex offender search was completed during the inspection. The central registry search has been processed. The results of the central registry will be forwarded to the licensing inspector upon receipt. In the future the administration will ensure all the correct out-of-state background checks are processed from to employment.
Standard #: 8VAC20-780-510-G Description: Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. One child's prescription medication was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. The medication has been at the center for three weeks. Plan of Correction: The parents have been advised to provide the prescription label for the medication.
