Comments:
An unannounced, on-site monitoring inspection was initiated on December 10, 2025 and completed on December 10, 2025. The on-site inspection began at 3:56pm and ended at 4:55pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be on 12/19/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-270-A
Description: Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. The following physical plant safety and maintenance issues were observed: There was loose wall boarder broken off the wall on the floor in the school age classroom.

Plan of Correction: Response from the center:
The base board was not broken off. I purchased it to replace the one on the opposite wall and the person who did our floors did not use it. I did not realize it was left in the room.

Standard #: 8VAC20-780-550-G
Description: Documentation shall be maintained of shelter-in-place and lockdown drills. The center did not document the lockdown and shelter in place drill.

Plan of Correction: Response from the center:
The staff was directed to the correct place to record the drills.
