An unannounced, on-site monitoring inspection was initiated on 12/15/2025 and completed on 12/15/2025, as a part of the licensure period. The on-site inspection began at 11:57AM and ended at 2:12PM. The inspector reviewed compliance in the areas listed above. There were 16 children present and 4 staff. The inspector reviewed 3 children?s records on site on 12/15/2025 and 3 staff records on site 12/15/2025. This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: REPEAT VIOLATION
The center is required to have employees undergo a background check every five years. The central registry and criminal background checks for Board Member #2 were overdue by 2 months.Plan of Correction: Provider will obtain current backgrounds.
Standard #: 8VAC20-780-160-C Description: A tuberculosis (TB) screening is required every two years from the last screening. Staff #1 updated screening was overdue by 2 months. Plan of Correction: Provider will obtain a follow up TB screening.
Standard #: 8VAC20-780-90-A Description: A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The record for Child #1, Child #2 and Child #3 did not have documentation of the agreement signed by parent. Plan of Correction: Provider will have agreements signed by parents.
Standard #: 8VAC20-780-540-C Description: First aid kits shall contain all required items. The first aid kits in both preschool buildings were missing several required items. Plan of Correction: Provider will ensure all first aid kits are stocked with required items.
