An unannounced, on-site focused inspection was initiated on 12/15/2025 and completed on 12/15/2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to supervision and special care provisions and emergencies.The on-site inspection began at 10:20 am and ended at 10:41 am. The inspector reviewed compliance in the areas listed above.
