During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 12/17/25. The on-site inspection began at 9:05am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 75 children present and 14 staff. The inspector reviewed 7 children's records and 8 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff 1 began employment prior to the center receiving the results of their national criminal background check.

Plan of Correction: The center responded with the following: Going forward, the school administration will ensure that staff do not begin any form of employment until the results of the national fingerprint search have been received.

Standard #: 22.1-289.035-B-3
Description: The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, who had been employed for six months, did not have a central registry search requested.

Plan of Correction: The center responded with the following: It appears there was a payment processing error when staff 1's central registry search was requested and the search was not completed. The central registry search for staff 1 has now been completed and the results have been placed in the staff record and forwarded to the licensing inspector.

Standard #: 22.1-289.035-B-4
Description: Repeat Violation
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff 2, employed for a year and a half, did not have documentation of requesting a central registry check from one state.

Plan of Correction: The center responded with the following: The out-of-state central registry search for staff 2 will be completed and results will be placed in the staff record and forwarded to the licensing inspector once received.

Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record for child 1, who had been enrolled for six months.

Plan of Correction: The center responded with the following: School administration will contact the parent to obtain a copy of the child's most recent physical.

Standard #: 8VAC20-780-160-C
Description: A tuberculosis (TB) screening is required every two years from the last screening. Staff 3's updated screening was due 4 months ago and has not yet been completed.

Plan of Correction: The center responded with the following: Staff 3 will obtain an updated TB screening this evening and documentation will be placed in the staff record.

Standard #: 8VAC20-780-240-A
Description: Repeat Violation
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff 2, who is working with children, has not yet completed this course which is overdue by over a year.

Plan of Correction: The center responded with the following: Staff 2 will complete the VDOE-sponsored orientation course as soon as possible and documentation will be placed in the staff record.
