Comments:
An unannounced monitoring inspection was conducted from 10:30 am - 11:50 am. There were 16 children in care being supervised by two staff members. The children were observed participating in cooperative games and preparing for lunch. Lunch is provided from home. Eight children's records were reviewed and in compliance. Two staff records were reviewed. Two medications were reviewed and in compliance. The most recent emergency escape drill was conducted 6/6/2017. Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of two staff records it was determined that not all criminal history checks were repeated every three years. Evidence: The most recent criminal history check for staff A was dated 05/01/2014.

Plan of Correction: Administration has requested the background check for staff A but has not yet received the results.
