8VAC20-780 Special Care Provisions and Emergencies
During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated on 11/17/2025 and completed on (12/19/2025 with local agencies, in response to a complaint received by the licensing office on 11/3/& 11/14/25 relating to allegations of programs and special care and provisions and emergencies. The inspector reviewed one child?s record and one staff records on-site on 12/17/25. This inspection included:

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/30/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-410-1 Complaint related: Yes Description: The following acts are forbidden: roughly handling, restricting movement through binding.
On several occasions, staff #1, was observed on video roughly handling child #1 (age 2). Staff #1 was also observed on video restricting movement by placing child #1 (age 2) arms beside their body and wrapping the child?s blanket around to prevent movement. Staff #1 was observed on video putting their leg on top of child #1 to prevent the child from moving while on the nap mat.Plan of Correction: Not available online. Standard #: 8VAC20-780-510-L Complaint related: Yes Description: Staff?s personal medication shall be kept in a locked place using a safe locking method that prevents access by children.
Child #1 (age 2) found a pill on the floor and put it in their mouth. The pill was not swallowed. The center took the pill to a pharmacy, and it was confirmed that it was a blood pressure pill.Plan of Correction: Not available online. Contact Inspector for more information.
