During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

An unannounced, on-site monitoring inspection was initiated on 12/16/2025 and completed on 12/21/2025. The on-site inspection began at 8:20 AM and ended at 9:50 AM. The inspector reviewed compliance in the areas listed above. There were 48 children present and 10 staff. The inspector reviewed 4 children?s records, 5 staff on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-770-70-A Description: Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
Staff #4, who has been actively working at the center for the past month, the two required references on file were not dated.Plan of Correction: The references were dated and in the future all forms will be dates as required.
Standard #: 8VAC20-780-210-A Description: Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent.
Staff # 1 with the job title of program lead did no have that they had fulfilled a high school program completion or equivalent.Plan of Correction: The diploma will be placed in the file for the staff person.
