An announced, on-site initial inspection was initiated and completed on 12/22/2025, as part of the application process for licensure. The on-site inspection began at 10:00 am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and 7 staff. The inspector reviewed 10 staff records and 8 children?s records on-site.
This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/29/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-150-B Description: Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
Child A, enrolled for 3 months did not have immunizations signed by a physician, his designee, or an official of a local health department.Plan of Correction: Family has been contacted to have doctor sign the immunization sheet.
Standard #: 8VAC20-780-70 Description: Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
The record for Staff 1, employed for 1 year 10 months, did not have documentation of completed reference checks.Plan of Correction: Staff member's references will be called by the director and filled. No staff member will be able to start without the completion of references.
Standard #: 8VAC20-780-240-C Description: Staff shall complete orientation training on required topics prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff 1, who has been employed for 1 year and 10 months, has no documentation of orientation available for review.Plan of Correction: Due to ownership transfer all staff have completed a new orientation under the new company and a certificate is on file.
Standard #: 8VAC20-780-330-B Description: Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus.
The fall zones on the school age playground had about 1 ? inch of resilient surfacing of mulch.Plan of Correction: Mulch has been ordered and will come on 12/26 to be installed on playground. Fall zones will be measured to ensure compliance.
Standard #: 8VAC20-780-510-B Description: Prescription and non-prescription medication shall be given to a child: only with written authorization from the parent.
There was no parental authorization for a prescription medication for Child 1.Plan of Correction: Parent will sign authorization upon return from winter break in order for student to return to school.
Standard #: 8VAC20-780-560-G Description: When food is brought from home, the following shall apply: The food container shall be sealed and clearly dated and labeled in a way that identifies the owner.
2 lunch bags in Junior Kindergarten and 2 lunch bags in the school age room had no dates.Plan of Correction: Staff have been informed of this and will apply a tape labeled properly with the information of children that bring lunches.
