An unannounced, on-site monitoring inspection was completed on 12/15/2025. The on-site inspection began at 10:22 a.m. and ended at 3:40 p.m. The inspector reviewed compliance in the areas listed above. There were 125 children present and 25 staff. The inspector reviewed 10 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/31/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: Repeat Violation
The center is required to obtain background checks from any state in which the individual
has resided in the preceding five years. S2, employed for 3 months, did not have
documentation of requesting a central registry check from one state.Plan of Correction: I will include all out of state background checks.
Standard #: 8VAC20-780-240-B Description: Repeat Violation
Staff shall complete orientation training prior to working alone with children and no later than seven days to include policies and procedures listed in 8VAC-780-420-A. The record for S2 did not contain the orientation training form showing training required by 8VAC-780-420-A.Plan of Correction: I will make sure all new employees will have the correct VDOE orientation form.
Standard #: 8VAC20-780-550-P Description: The center shall maintain written records of children?s injuries that contain all of the
required information. Injury reports completed in November 2025 did not document the time of parental notification regarding the injuries. The reports documented the notifications at ?pick up.?Plan of Correction: I will go over with all teachers to make sure they include the time of pick up and
not just say "pick up".
