Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated on December 17 2025 and completed on December 17 2025. The on-site inspection began at 12:30pm and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 67 children present and 13 staff. The inspector reviewed 4 children?s records and 2 staff records. This inspection included a document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 7 2026. A POC submitted after this date will not appear on the public website.

Standard #:	22.1-289.035-B-4
Description:	Authorize the child day center to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. The center did not submit a request for a criminal record check, a sex offender registry check, and a central registry check for all states Staff #1 resided in within the last 5 years. The center is 3 months out of compliance.

Standard #:	8VAC20-780-160-A
Description:	Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening.The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. The center did not ensure that the Tuberculosis screening received for Staff #1 was within 30 days of the employment date. The screening is 14 months older than required.

Description:	Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Staff #1 and Staff #2 do not have documentation that orientation was conducted or received. The center is 3 months out of compliance in completing new staff orientation for Staff #1 (start date 9/12/25) and Staff #2 (start date 9/5/2025).
