Comments:
An unannounced, on-site monitoring inspection was completed on 11/20/2025. The on-site inspection began at 10:20 a.m. and ended at 4:40 p.m. The inspector reviewed compliance in the areas listed above. There were 154 children present and 20 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/06/2026. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-C
Description: Repeat Violation
A tuberculosis (TB) screening is required every two years from the last screening. The TB screening for S8 was completed 8 days late.

Plan of Correction: NA

Standard #: 8VAC20-780-290-A-3
Description: In areas used by preschool age or younger children, electrical outlets shall have protective covers. A power strip was observed in Building 2 under the water fountain that did not have protective covers.

Plan of Correction: Power strip was immediately removed from the common area.

Standard #: 8VAC20-780-420-E-3
Description: Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of C1 had not been updated in over 22 months. which is overdue by 10 months.

Plan of Correction: Audit all children files to make no other families have missed the update to their information.

Standard #: 8VAC20-780-510-B
Description: Prescription and nonprescription medication shall only be given to a child with written authorization from the parent. Staff in Room 2 reported administering a non-prescription ointment for C17 earlier in the day. The ointment was provided by the parent; however, no written permission was completed.

Plan of Correction: Retrained faculty on ointment applications.

Description: When food is brought from home the container must be clearly labeled with the child?s name. Multiple thermos bottles were observed in the Rising Rockets classroom were not labeled with the children?s names.

Plan of Correction: Will send reminders to parents to double check that they have labeled their child's containers and have reminded all teachers to check these items upon arrival in the morning.

Description: Repeat Violation
When a feeding table is being used for children under 12 months, the protective belt shall be fastened securely. Two infants were observed at a feeding table in the Cuddly Cabooses Room without a protective belt being securely fastened.

Plan of Correction: Retrained teachers on making sure the infants are always secured.
