During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced monitoring inspection was initiated on December 19, 2025, and completed on January 23, 2026, in response to a self-report received by the licensing office on December 19, 2025, relating to Staffing and Supervision, Special Care Provisions and Emergencies and Special Services. The inspector reviewed 1 child record and 4 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to Special Care Provisions and Emergencies and Special Services and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on January 30, 2026. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-4
Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry from one state for Staff #1, who has been employed for over four months.

Plan of Correction: The Center's method of checks and balances has been revised to prevent missing background checks from any required states.

Standard #: 8VAC20-780-550-I
Description: A 911 or local dial number for police, fire and emergency medical services shall be posted in a visible place at each telephone.

The number for 911 or local dial number for police, fire and emergency medical services were not posted in a visible place near the two telephones in the center.

Plan of Correction: Information for local emergency services are posted next to telephones in every classroom. Staff are to check for the post daily incase they have been dislodged.

Standard #: 8VAC20-780-560-F
Description: When centers choose to provide meals or snacks, a menu listing foods to be served during the current one-week period shall be posted in a location conspicuous to parents or given to parents.

A snack menu for the current one-week period was not posted or given to parents. Staff #2 confirmed that the snack menu was not posted or given to parents.

Plan of Correction: Parents will continue to be notified on the weekly snack menu through BSLC website and classroom postings. Parents will be notified of any changes before substitutions are served.

Standard #: 8VAC20-780-560-F-4
Description: When centers choose to provide meals or snacks children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

Children in classroom #1 (ages: 2-3 years) were offered string cheese sticks that were cut in half making a cylinder shape approximately 2 ? inches long and approximately ? inch wide.

Plan of Correction: String cheese has been removed from the menu and replaced with shredded cheese. Staff have received training on potential choking hazards and reminders are posted in each classroom.
