During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 12/29/25. The on-site inspection began at 10:05am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 77 children present and 14 staff. The inspector reviewed 7 children's records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: The center is required to have employees undergo a background check every five years. The most recent central registry check for staff 2 is overdue by two months.

Plan of Correction: The center responded with the following: The repeat central registry check for staff 2 has been requested. Once received, the results will be forwarded to the licensing inspector and placed in the staff record.

Standard #: 22.1-289.035-B-4
Description: Repeat Violation
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff 3, employed for one year, did not have documentation of requesting a central registry check or a sex offender registry check from the state they previously resided in.

Plan of Correction: The center responded with the following: The out-of-state background checks for staff 3 have been requested. Once received, the results will be forwarded to the licensing inspector and placed in the staff record. Going forward, center management will ensure that all background checks are requested within the required timeframes.

Standard #: 8VAC20-780-130-A
Description: Repeat Violation
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunization records for child 1 was obtained over a week after they began attending the center.

Plan of Correction: The center responded with the following: Going forward, center management will ensure that documentation of immunizations is obtained by the first day of attendance. Management will conduct regular audits of files to ensure compliance with the standards.

Standard #: 8VAC20-780-130-E
Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Documentation of additional immunizations for child 1 is overdue by one month.

Plan of Correction: The center responded with the following: The updated immunization record for child 1 was obtained during the inspection.

Standard #: 8VAC20-780-70
Description: Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff 1, who has been actively working at the center for over 3 months, did not have any completed reference verifications on file.

Plan of Correction: The center responded with the following: Staff 1 was a transfer from another center and references were not forwarded with the record. The center director obtained references and they have been placed in the staff record. Going foward, center management will audit records for transfer staff once received to ensure they contain all required information.

Standard #: 8VAC20-780-550-G
Description: Documentation of emergency practice drills shall contain all of the required elements. The practice drill log for the center does not include the method used for notification of the drill, the number of staff participating, any special conditions simulated, problems encountered, if any, or the weather conditions.

Plan of Correction: The center responded with the following: The correct practice drill log has been printed out and will be used going forward.
