During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on December 22, 2025 and completed on December 22, 2025. The on-site inspection began at 11:00 AM and ended at 2:00 PM. The inspector reviewed compliance in the areas listed above. There were 83 children present and 15 staff. The inspector reviewed eight children?s records and eight staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on January 7, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
1. In room #8, plastic paneling on wall is warped and cracked.
2. In bathroom #1, one sink does not have access to warm water.
3. In bathroom #2, one sink does not have access to warm water.Plan of Correction: We had work orders submitted for the violations. Revisit monthly to ensure completion.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
1. In room #3 and room #4, chemicals were stored in unlocked closets.
2. In room #6, chemicals were stored in an unlocked cabinet.Plan of Correction: The cook is going to inspect each room during meal delivery to ensure no chemicals are unlocked.
Standard #: 8VAC20-780-510-L Description: Medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
In room #8, two over the counter medications were located on top of a cabinet in the bathroom, unlocked.Plan of Correction: Remind staff member they must lock up their child's medicine. Using locked closet now.
