An unannounced monitoring inspection was initiated on 11/17/2025 and completed on 1/5/2026 with the Smyth County Department of Social Services in response to a self-report received by the licensing office on 11/11/2025 relating to Administration, Staffing and Supervision, Programs and Child Abuse and Neglect. The inspector reviewed one child?s record and four staff records. This inspection included document review, interviews and observation.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
