An unannounced monitoring inspection was initiated on 01/06/2026 and completed on 01/12/2026 in response to a self-report received by the licensing office on 12/30/2025 relating to programs. The inspector reviewed 1 children?s record and 1 staff record on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

On 12/302025, Staff 1 struck Child 1 (age 17 months).Plan of Correction: Staff has been terminated and provider will conduct staff training.
