During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
The Licensing Inspector discussed General Procedures, OBI portal and background checks, child records, and allergy management.

Comments:
An unannounced monitoring inspection was conducted on 1/8/2025 from 11:05am to 12:55pm. There were 36 children and 12 staff present. The inspector reviewed 5 child records and 2 staff records on-site. The inspection included document review, interviews, observations, and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/20/2026. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled which contains the following: place of employment and work phone number for each parent who has custody.

Two out of five child records reviewed did not contain complete employment information for each parent. On the date of the inspection, Child #1 had been enrolled for six months. Child #2 had been enrolled for four months.

Plan of Correction: When parents leave employment lines blank due to self employment, or can only provide cell not work number, they will be asked to write NA on form. The 2 parents in question were contacted on 1/8/2026 to confirm NA should be written on form.
