Discussed creating a comprehensive data collection tool to accurately document who, what, why, when and where in order to identify any patterns or triggers.

Discussed creating a concrete plan that details interventions to ensure all teachers are addressing the aggressive behaviors consistently and intervention successes and failures can be assessed.

Comments:
An unannounced inspection was initiated on 1/13/2026 and completed on 1/26/2026 in response to a complaint received by the licensing office on 1/8/2026 relating to allegations of care and protection of children. This inspection included
? document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms),
? tour of the facility
? interviews
? observations (this would include observations of photos, recordings, etc.)

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/4/2026. A POC submitted after this date will not appear on the public website.

Complaint related: Yes
Description: The center is required to maintain compliance with the center's own policies and procedures that are required by these standards.

The center's parent handbook indicates that "should a child be involved in an incident during the course of the school day, a staff member will complete an incident report." Since November 2025 Child #1 has exhibited biting behavior towards center staff and other children several times a day. These incidents were not documented on incident reports.

The center's parent handbook indicates that "when we experience ongoing biting, we develop a plan of action with strategies, techniques, and timelines to work on the problem." As of 1/13/2026 a plan of action had not been developed.

Plan of Correction: The center's policy of completing incident reports as well as accident reports has been reviewed with the caregivers/teachers and is being followed.

The center's policies to develop a plan of action with strategies, techniques and timelines to work on the problem was reviewed with caregivers/teachers and the child's parents as described above beginning 13 January (caregivers/teachers) and 14 January (parents) and is currently being reviewed, reflected upon and revised as needed/indicated in serving the child successfully.

Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.

The center did not ensure the care and protection of the other children in Classroom A. Since November 2025 several children have been bitten and had their hair pulled by another child. Staff did not have access to interventions and additional support staff to keep the children safe.

Plan of Correction: While working to ensure the health, safety, welfare and well-being of all children enrolled as well as their caregivers/teachers several children in classroom A were bitten and several children had their hair pulled.
In response, to identify and address circumstances/contributing factors unscheduled conversations with caregivers/teachers were engaged as incidents occurred and were reported with impromptu follow-up conversations to review and reflect on the success (or lack thereof) of those strategies to develop informed modifications which were also implemented and assessed. Implementation plans included administrators reporting to the care and early learning setting to offer support and assistance.
To better ensure the health, safety, welfare and well-being of all children enrolled as well as their caregivers/teachers in classroom A, a meeting between two of the caregivers/teachers and two members of the administrative team was held 13 January and since to discuss and develop more concrete strategies for identifying and addressing circumstances/factors contributing to the child's dysregulated/impulsive behavior. At meetings strategies are reviewed and reflected upon with the intention of developing and refining strategies that are successful and aligning the strategies being implemented at the center as strategies to be used at home as well as vice-a-vie.
As mentioned at the time of the inspection, a meeting with parents was scheduled for 14 January. The purpose of that meeting (which did take place as scheduled) was to have an update on the result of steps the family had initiated in response to their child's behavior, receive information/updates shared by the caregivers/teachers to discuss and design more concrete strategies and to define the circumstances under which in response to dysregulated and harmful behaviors, the parents would be asked to come to the center to take the child home for the remainder of the day. This plan was implemented 14 January, remains in place to date and will remain in place as long as it is needed and proved successful. Subsequent meetings have been held with the parents.

Complaint related: Yes
Description: The center did not maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. Incidents of minor injuries were reported verbally by staff but the injuries were not documented. Incidents have been occurring since November 2025.

Plan of Correction: Since the time of the inspection, written records of children's injuries are being/have been documented.
