During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 1/14/2026. The on-site inspection began at 10:00 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 15 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, observations, and interviews.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 1/22/2026. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry or sex offender registry from one state for Staff #3. Plan of Correction: Make sure all background checks are done prior to employment.
Standard #: 8VAC20-780-160-A Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #3 TB was dated 5 days after the date of employment. Plan of Correction: Make sure TB's are done prior to employment.
Standard #: 8VAC20-780-70 Description: Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #3 completed reference verifications on file were dated eight days after date of employment. Plan of Correction: Make sure references are done prior to employment.
Standard #: 8VAC20-780-380-A Description: There shall be a posted daily schedule. There was no daily schedule posted in one of the classrooms. Plan of Correction: Posted daily schedule.
