Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site renewal inspection was initiated and completed on 01/04/2026. The on-site inspection began at 10:15 am and ended at 1:21 pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 17 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard #:	22.1-289.035-B-3
Description:	The center must request a search of the central registry prior to the employee?s first day of employment. The central registry for Staff 2 was requested more than one week after the first day of employment.
Plan of Correction:	Moving forward, will request before hire; will correct.
Standard #:	8VAC20-780-270-A
Description:	Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. The exterior paint of the building that is adjacent to the playground is peeling; the peeling paint is on an exterior wall that is accessible to the children. There are various areas of peeling paint in Classroom 4; the peeling paint is accessible to the children.

Description:	REPEAT VIOLATION When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. The authorization for Medication 3 expired over 3 months ago; the medication was still on-site.
