During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on January 15, 2026 and completed on January 15, 2026. The on-site inspection began at 10:10 a.m. and ended at 11:30 a.m. The inspector reviewed compliance in the areas listed above. There were six children present and two staff. The inspector reviewed all existing staff records for updates and one new staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 27, 2026. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-260-B
Description: After the first license, annual approval from the health department shall be provided. The health inspection that was on record at the center was completed one year and six months prior.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. A spray bottle of Clean Smart cleaning solution was left on the sink counter by the changing table outside of diapering time during the entire inspection.

Plan of Correction: Lock installed on sink cabinet.

Description: SYSTEMIC DEFICIENCY The center shall implement a monthly practice evacuation drill. A practice evacuation drill was not conducted in December.

Standard #: 8VAC20-780-550-E
Description: REPEAT VIOLATION/SYSTEMIC DEFICIENCY Shelter in place procedures shall be practiced a minimum of twice per year. No shelter in place practice drill was conducted in 2025.
