Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Discussed timeline for correcting background check violations. Explained background check requirements for hiring new staff, medication labeling, and what to do when medication expires. Made recommendations on locating traffic cones when children are playing on the blacktop. Inspector will share additional information concerning background checks in an email.

Comments:

An unannounced, on-site renewal inspection was initiated and completed on 1/15/2026. The on-site inspection began at 10:45am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were fifty-one children present and ten staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-60-A
Description:	Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #4, was missing the work phone number of one parent who has custody.
Plan of Correction:	The center will obtain work phone numbers for the parents.
Standard #:	8VAC20-780-70
Description:	Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #2, who has been actively working at the center for the past seven days, did not have any completed reference verifications on file.
Plan of Correction:	The center will obtain two references for the staff,
Standard #:	8VAC20-780-270-A
Description:	The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Ground covering was exposed underneath the mulch on the playground, causing a tripping hazard. Paint was chipping on one of the metal climbing structures on the playground. The plastic coating on the chains on the swings was peeling off.
Plan of Correction:	The center will have the playground areas repaired.
