During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

An unannounced, on-site renewal inspection was initiated and completed on 1/29/26. The on-site inspection began at 4:30pm and ended at 5:50pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 3 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/6/26. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Repeat Violation
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information including, the name address and phone number of two designated people to call in an emergency if a parent cannot be reached. The record for Child #1, enrolled for five months, did not contain documentation of a second person, other than a parent, to contact in an emergency if a parent could not be reached.Plan of Correction: The facility responded: Staff have been trained on children files and ensuring each child has two designated people as emergency contacts.
Standard #: 8VAC20-790-600-F Description: Staff who work directly with children shall annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course. The record for Staff #1 (employed for 17 months) did not contain documentation that the 2025 annual health and safety update course was completed. Plan of Correction: The facility responded: The documentation of completion of the health and safety update course will be placed in the staff member record.
