During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be

An unannounced, on-site renewal inspection was initiated on 1/23/2026 and completed on 1/23/2026. The Department issued a Special Order on 8/18/2025 to contact parents, guardians, or other responsible persons regarding health and safety violations, as a result of high-risk violations. The on-site inspection began at 11:00 am and ended at 1:18 pm. The inspector reviewed compliance in the areas listed above. There were 28 children present and six staff. The inspector reviewed five children?s records and five staff records on-site on 1/23/2026. This inspection included document review, tour of the facility, interviews and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/6/2026. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Description: REPEAT VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment.
In the Pre-K classroom, rust was observed on the escutcheon around the water supply line and on the toilet paper holder in the girls? restroom.
In the school-aged classroom, rust was observed on the wall-mounted support frame surrounding the sink.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-500-B Description: Disposable diapers shall be disposed in a plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
The trash can lid in the Two-Year-Old Classroom was broken, and on the floor. There were soiled diapers/pull-ups in the trash can.Plan of Correction: Not available online. Contact Inspector for more information.
