During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on 12/22/25 and completed on 02/04/26, in response to a self-report received by the licensing office on 12/02/25 relating to supervision. The inspector reviewed one child?s record and one staff's training certificates on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to special care provisions and are documented on the violation notice issued to the program. Violations have been cited related to noncompliance resulting in an injury to a child while under the supervision of the facility that required outside medical treatment. Please see the violation notice for more details.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Standard #: 8VAC20-780-340-A Description: (Repeat Violation) When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 12/01/25 during morning snack, Child #1 (2-years-old) was sitting at a table with another child where an allergen was served. The child put her hands in the allergen and grabbed her cup. Child #1 touched the child?s cup and later touched his face. As a result, Child #1 suffered an allergic reaction, which included red, swollen eyes, lips, and itchiness. The child was administered emergency medications and was transported to a local emergency hospital.
Staff in the classroom were aware that Child #1 had a diagnosed food allergy, and no additional measures were taken to prevent cross contamination to ensure Child #1 was not near the allergen when it was served.Plan of Correction: Per the Center: "Child will be seated at the end of the table with space from peers, supervised directly by a teacher during meals. Child will be served last and dismissed first for handwashing.
Staff retrained on allergy supervision and cross-contamination prevention."
Standard #: 8VAC20-780-500-A Description: Children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.
Video footage reviewed and interviews conducted revealed that on 12/01/25, some of the children in the Twos classroom did not wash their hands after morning snack.Plan of Correction: Per the Center: "Teachers were retrained on required handwashing procedures and will ensure all children wash hands before and after meals/snacks."
