An unannounced, on-site monitoring inspection was initiated and completed on 2/18/2026. The Department entered into a Consent Agreement with the facility on 11/3/2025. The on-site inspection began at 10:40am and ended at 12:35pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and 7 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/25/2026. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-430-B Description: Materials and equipment available shall be age and stage appropriate for children.
The toddler class was using dried corn kernels in a craft activity and multiple children were putting the kernels into their mouths. The dried corn kernels pose a potential choking hazard.Plan of Correction: Craft was discontinued & kernels removed from the room.
Directors will continue the practice of asking to see weekly lesson plans and material requests ahead of time.
Directors have re-trained and reminded staff to be weary of choking hazards and specifically make sure toys and materials are safe and age-appropriate.
