An unannounced, on-site monitoring inspection was initiated and completed on 02/19/2026. The on-site inspection began at 10:48 AM and ended at 12:24 PM. The inspector reviewed compliance in the areas listed above. There were 35 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, observations, and measurements.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 02/26/2026. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-M Description: The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
The classrooms that had an allergy list were not current or dated. One classroom that did not have an allergy list had children present with a diagnosed allergy.Plan of Correction: The provider updated this after the inspector left the center.
Standard #: 8VAC20-780-510-P Description: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.
The authorization form for medication #3 expired 12/26/2025. The center administered the medication three times after the authorization form expired.Plan of Correction: The provider is getting the parents to sign the form now and going forward they are going to have a system in place to audit and stay on top of this.
