During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site focused inspection was initiated on March 10, 2026 and completed on March 10, 2026, as a part of probation. The Department issued a Special Order on February 10, 2026 to PLACE THE LICENSEE ON PROBATION AND REQUIRE LICENSEE TO CONTACT PARENTS, GUARDIANS, OR OTHER RESPONSIBLE PERSONS as a result of high-risk violations. The on-site inspection began at 10:00 a.m. and ended at 11:00 a.m. The inspector reviewed compliance in the areas listed above. There were ten children present and two staff. The inspector reviewed two children?s records and three staff records on-site. This inspection included observations and record review. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
